GUIDE (Guiding an Improved Dementia Experience)
Alternatives is excited to offer comprehensive dementia care services through its participation in the GUIDE Model from the Centers for Medicare & Medicaid Services (CMS). The GUIDE Model is a nationwide CMS Innovation Center program that funds comprehensive, coordinated dementia care with the goal of improving quality of life and helping people with dementia remain safely at home.

No cost: Fully covered for beneficiaries with Traditional Medicare A & B
Personalized care plan and navigation: Interdisciplinary assessment and ongoing navigation across
medical and community services.
A personal coach: Families are paired with a professional care navigator who communicates with you
regularly to help address the many challenges that can arise.
24/7 clinical help: A round-the-clock helpline for urgent questions or behavioral symptoms between
visits.
Medication Management: Ongoing reviews and coordination to help keep treatments appropriate and safer.
Caregiver support: Evidence-based education, coaching, and support groups.
Respite options: Short-term in-home, adult day, or facility-based respite so caregivers can take
needed breaks.
Enhances Existing Medical Providers: Alternatives is partnered with Eli Health to provide Dementia
focused clinical support. Eli Health’s medical group works alongside the patient’s existing medical providers, adding an extra layer of support.
